Welcome to the home page for the online forms required by UIL and Flour Bluff Independent School District.

This automated process allows us to be more efficient in handling the forms as well as saving valuable resources. These forms are required each year your son or daughter wishes to participate in athletics for FBISD. You will need to fill out the forms and submit them in a timely manner in order for your son/daughter to be allowed to participate. Students are required to use the Pre-participation- Physical Examination Form provided. NO OTHER PHYSICAL EXAM FORM WILL BE ACCEPTED. A new physical is required each year for all student athletes, prior to the start of the new school year.

1. The required annual athletic physical and health history cannot be filled out online because it must be completed by the parent/guardian and the physician. You will need to print a physical and medical history form by selecting the physical and medical history tab at the top of the page under the “download and print tab”. Once you have selected the tab you will see the link to the physical and medical history form and you will be able to download and print. You are encouraged to fill out the medical history prior to printing the form. The physical exam will be the only form you will need to print and take to the physician to be completed.

2. All other required paperwork should be completed electronically. As a parent/guardian, you will be asked to provide information such as home phone numbers, cell phone numbers, email addresses, name of the primary physician, and other pertinent information. Please make sure you collect this information prior to filling out your forms electronically.

3. You will not be able to save information before submitting. To submit online appropriately, you and your child are required to sign in all designated spaces. Forms will not be accepted electronically without all required information being completed.

4. To access the required athletic participation forms for Flour Bluff ISD, hold your cursor over the “Electronic Participation Form” tab. You will get a dropdown list of all required forms.

5. To begin, click on the form name and fill out the information requested. Parent/Guardian must have their son/daughter present along with the student ID number available to complete the forms. If you do not know the answer to a specific question, please answer the question with N/A, or none.

6. To sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using an IPAD, you may create an electronic signature by holding your finger on the screen and sign your name or by using a stylus.

7. Once you have completed all of the information on each page, you will have the opportunity to print the document.

8. You will receive a confirmation email once the document(s) has been reviewed by FBISD Athletic Training Staff.

9. If your son/daughter requires the use of an inhaler or EpiPen, the appropriate FBISD form must be printed from the downloadable forms section. The form must be completed by a physician. Once the physician has completed and signed, submit the form to the athletic trainer along with the physical exam form. You must present the inhaler or EpiPen to the Athletic Training Staff prior to your child’s participation in the sport.
